What Else You Should Know…

Plans cannot be added to additional insurance plans or discounts.
Payment for any treatment not included in the membership plan is due at the time of service. A 15% discount will be applied if paying with cash or check. A 10% discount will be applied if paying by credit/debit card.
Plan payments are non-refundable. Plan is based on 12 revolving months.
Plan is subject to change yearly.
The membership plan does not cover procedures completed by a specialist outside of our practice.
Plans and copays cannot be paid by Care Credit.
